Lap Partial Nx DOS Predictor

Platform: This is a program for Windows™ PC based computers.
Introduction and Objective: An algorithm to preoperatively predict post laparoscopic partial nephrectomy hospital duration of stay (DOS). This could be utilized to streamline the delivery of care during the hospital stay.
Methods: Retrospective review of all patients (334) who underwent laparoscopic partial nephrectomy by two surgeons between September 1999 and April 2004 at our institution. Preoperative and intraoperative findings were recorded. Two algorithms were designed to predict if the DOS would be less than or greater than: (i) two days, and (ii) one day. Univariate and multivariate logistic regression analysis identified significant preoperative predictors of DOS: age, hypertension, reflux disease, anxiety, hematuria, diabetes, renal insufficiency, gout, congestive heart failure, Crohn’s disease, planned transabdominal approach, solitary kidney, size of tumor by CT, exophytic tumor, tumor up to renal sinus and tumor abutting the collecting system. These factors were then used to create linear prediction algorithms.
Results: Accuracy was 73% (area under ROC 0.7) in predicting a DOS less than or greater than two days. Accuracy was 83% (area under ROC 0.8) in predicting a DOS less than or greater than one day.
